A SmilePass membership is your practice's own care plan: patients pay a small regular amount for their preventive care plus a discount on other treatment. There is no one-size-fits-all version. This short guide shows every choice you'll make, so you can walk into the builder knowing exactly what you want.
Choice 1
What should it feel like?
The experience you want patients to associate with your membership. This is separate from how the tiers are organised, which comes next.
Discount club
Savings-led. Patients join mainly to get a discount on treatment. Simple and transactional.
Care plan
Health-led. Framed as a structured ongoing-care commitment, with regular prevention at the centre. Builds the strongest loyalty.
VIP / premium
Experience-led. Priority, perks and a sense of exclusivity. Suits cosmetic, implant and boutique practices.
Mix
A blend: a care-plan backbone, savings throughout, and a premium top level. The most common choice.

How should the tiers be structured?
This decides how your membership levels are organised: patients choose between these levels. Pick the shape that fits how your practice works. Not sure? Good, Better, Best suits most practices.
Good, Better, Best
Suits most practicesThree value levels from basic to premium, with the middle level as the one you recommend to most patients. Each step up adds more inclusions and a bigger treatment discount.
Example: Essential $5/wk (2 cleans) · Standard $9/wk (2 cleans, 2 exams, x-rays) · Premium $14/wk (adds whitening, priority booking and a bigger discount).
By clinical risk
Clinically defensibleLevels are set by each patient's clinical risk. Lower-risk patients need fewer visits and pay less; higher-risk patients get more frequent care. Priced on clinical need, so it is easy to justify.
Example: Low Risk $7/wk (1 exam, 1 clean) · Average Risk $11/wk (2 exams, 2 cleans) · High Risk $22/wk (2 exams, 4 cleans, gum maintenance).
By treatment pathway
For clear patient cohortsA separate maintenance plan for each treatment journey you offer. Each patient joins the plan that matches their care. Best when you have clear groups such as implant, orthodontic or cosmetic patients.
Example: Preventive Maintenance $7/wk · Implant Maintenance $8/wk · Cosmetic Maintenance $12/wk. Each patient picks the plan that fits their treatment.
Discount only
Lowest adminNo included visits. Members pay an annual fee and get a discount on all treatment. The simplest option with the least admin, built around savings rather than bundled care.
Example: Annual Savings Plan $199 per year gives members 15% off all treatment. No cleans are bundled in.

The building block
Inside every tier
Whatever structure you choose, each tier is built from the same parts, and every one of them is yours to edit before anything goes live.
Tier name
What patients see on the plan. Short and easy to say at the front desk.
Price per week
Seeded from your check-up and clean fee with a member saving applied, spread across 52 weeks. Fully adjustable.
Setup fee
An optional one-off joining fee. Most practices keep it at $0.
Member discount %
The discount on treatment outside the inclusions. It rises up the ladder, for example 5%, 8%, 10%.
Billing frequency
Weekly, fortnightly, monthly or annually. Offer one option or several.

The care included each year: exams, cleans, x-rays, fluoride, screenings. ADA item numbers shown where they map.

The fine print, such as a waiting period before the first included clean.

Add-ons are optional extras members can buy on top of their tier, priced per week just like the tiers. Perks are the soft benefits that make membership feel like membership; they cost nothing extra and are listed on the plan.

Check-up & clean saving
15%How much a member saves on a regular check-up and clean versus paying visit by visit. It sets every tier's weekly price: higher saving, lower weekly price and a bigger reason to join; lower saving, more margin per member.
Family discount
10%A percentage off for each additional family member on the plan, making it easy to sign up the whole household.
Referral active
12 moHow long a referral reward stays active after a member refers someone new to the practice.
Waiting period
3 moHow long a new member waits before their first included clean. Protects against join-use-cancel behaviour.
